TELUS Community Care Management Solution (CCMS), powered by HSAGlobal, is a full electronic health record, patient care management and business management software solution. It enables community care providers to send and receive secure patient information at the point of care, using a standard Microsoft interface on a laptop, PDA or smart phone. Updates can be made to assessments and care plans, test results can be accessed, and clinical processes reviewed at the time of patient care. This critical information can then be shared across the continuum of care through secure access with other healthcare providers. All data resides in secure data centres that meet all provincial and national security and privacy standards.

Our solution uses a common language for assessing health status and care needs based on the Home Care assessment system, or HC. The HC system is used in many countries around the world as a standard in community care We support the latest version (interRAI HC) with our electronic InterRAI-HC module. It uses common measures to enable clinicians and providers in different care settings to improve continuity of care, as well as to integrate care plans and support for each individual. This critical information can then be shared across the continuum of care through secure access with other healthcare providers. All data resides in secure data centres that meet all provincial and national security and privacy standards.
