TELUS CareShare™ is the glue that allows providers to access and share the information with other providers in order to manage their patients. Workflow is layered on top of the data so the providers are informed of progress and can provide comprehensive, timely care to the patient.

CareShare is offered through a Software as a Service (SaaS) or Cloud Computing model where users don’t have to deal with systems and interfaces; they simply subscribe to a service and connect to the cloud and use the available services which include:

• Results Distribution – enables the sharing of patient results amongst multiple providers, such as from acute care to primary care of private labs to primary care
• eReferral - offers an ecosystem for use by primary care and specialist physicians to facilitate the sharing of electronic referrals and patient information
• Medication Reconciliation – aggregates medication data for patients from a variety of available data stores and provides the healthcare team with tools and workflow processes to review and reconcile those medications.
• Care Coordination and Remote Patient Monitoring - supports coordinated patient home care process and enables patients to stay in touch with their healthcare providers

The TELUS CareShare™ data set includes real-time information aggregated into services from many sources such as:

1. Pharmacy/Lab Systems
2. Patients (through TELUS health space)
3. Health Information Systems
4. Electronic Medical Record Systems
5. Jurisdictional Electronic Health Records
6. Acute care facilities

Data is not stored in CareShare. It is viewed from the source systems as required.

TELUS CareShare™ represents the SaaS model for offering these solutions. They are also available in a Managed Services model, where the applications and data are managed by TELUS, for those health delivery organizations that required customized workflows and want to run the functionality specifically for their users or in a Licensed Services model for those organizations requiring customized functionality, infrastructure and application control implemented and managed by the client.
